For rural Massachusetts health centers and clinics
Massachusetts is funding remote patient monitoring for rural providers. Applications due Monday, Oct 26.
Up to $200,000 per participating organization. Applications close Monday, Oct 26, 2026 at 5:00 PM ET.
Long Haven Partners can help your team put together a strong application this week. Your organization stays the applicant. We bring NestCara, a health-at-home platform that runs on the TV your patients already own.
The Massachusetts problem
You already know the pattern. A patient with diabetes, high blood pressure, heart disease, or COPD leaves a visit with a good plan. Then they go home, often many miles from the clinic, and weeks pass before anyone sees how they are doing.
The state names the same barriers in its own words: geographic isolation, transportation limits, workforce shortages, limited primary and specialty care, a higher chronic disease burden, and financial pressure on rural providers. Many rural organizations also lack the staff, technology, and resources to expand services. Health centers that serve large numbers of MassHealth members feel that pressure most.
The gaps between visits are where problems grow. A blood pressure creeps up. A patient skips pills. Nobody knows until it turns into an ER trip.
The state now has a program and money to close that gap
The Massachusetts Rural Health Transformation Program (RHTP) is a five-year, $800 million federal investment in rural health. Its current Rural Partners round includes Activity 1.5, Expand Remote Patient Monitoring. The goal is to help rural providers watch chronic conditions at home, step in earlier, and reduce avoidable ER and hospital use.
Who can apply for RPM: rural hospitals, rural FQHCs, Rural Health Clinics, and rural-located primary care practices. You do not need an RPM program today. The state says organizations can be at different stages of readiness. The first participation period runs Oct 30, 2026 through Sep 30, 2027.
NestCara: monitoring that fits into patients' lives
Most RPM programs ask older patients to learn a new tablet or app. Many never do. NestCara takes a different path. A small box plugs into the TV in the living room, and the TV becomes the patient's link to your care team.
- Nothing new to learnPatients use the TV and remote they already know, plus their voice.
- Daily check-insThe TV asks how the patient feels. NestCara notes changes from that person's normal.
- Passive home sensors and health readingsHome sensors work in the background. A blood pressure cuff, scale, and blood oxygen meter can send readings on their own.
- Built for care teamsYour staff use a web portal with alerts sorted by urgency, video visits on the patient's TV, and readings ready to support the patient's record (EHR).
- Consent comes firstPatients choose who sees their data. Each person on the care team gets only the part they need. The camera and mic have switches on the box.
Family can stay in the loop through a simple phone app, so the patient is not alone between visits. NestCara is patent pending.
NestCara does not replace 911. In an emergency, call 911.
How we help
We help you build the application. You stay in charge.
One week is tight for a small team that is already stretched. Long Haven Partners works alongside your staff to get a complete application ready on time.
- A 20-minute call. We learn about your patients, your region, and your goals for RPM.
- Drafting support. We help your team prepare the application form (Attachment A), including the RPM questions and, if they fit you, Chronic Disease Management Networks or Home Visiting.
- Partnership form. Long Haven Partners is named as your technology partner on the Partnership Validation Form (Attachment B). The state notes that signing this form does not create a contract.
- You submit. Your organization reviews, signs, and submits through COMMBUYS. We aim to finish by Friday, Oct 23, to leave room before the deadline.
Your organization is the applicant, and any award goes to you. Your team decides what goes in and what gets sent.
Decide by Wednesday, Oct 21
A strong application needs a few working days. If remote patient monitoring belongs in your plans, let's talk this week.
Final deadline: Monday, Oct 26, 2026 at 5:00 PM ET, through COMMBUYS.
